Medical Forms
1. Youth campers and youth staff must have the following documented up-to-date, according to the recommended U.S. Centers for Disease Control and Prevention (CDC) vaccination schedule by age:
- Diptheria, Tetanus, and Pertussis vaccine (DTaP, DTP, TDaP)
- Hepatitis B vaccine or proof of immunity
- Measles, Mumps, and Rubella vaccine or proof of immunity
- Polio vaccine (IPV, OPV)
- Varicella (Chicken Pox) vaccine or proof of immunity
- Meningococcal conjugate vaccine (MenACWY)

2. What documentation is accepted for "proof of the measles vaccine"?
Confirm measles immunity for all youth campers and staff by receiving written record of:
- two doses of MMR (measles, mumps, and rubella vaccine);
- laboratory test showing measles immunity;
- laboratory confirmation of measles;
3. We require that Scouts and Scouters atched doctor supplied Vaccination records to meet Sulvain County Documentation requirment. Part 2B of the Medical Form "Immunizations" does not meet Sulliavan Counties Requiremnets.
