FSR Summer Camp Forms & Important Information

Welcome to the FSR Summer Camp Forms page!

Here you’ll find everything you need to complete before arriving at camp, including merit badge selection, financial assistance options, health forms, and more.

Program Registration

To help Scouts select their merit badges, use the Personal Program Form found in the Leader's Guide. Scouts should:

Review merit badge prerequisites before selecting badges.
Ensure they meet the prerequisites before arriving at camp.

FSR Campership Program (Closes April 15th) Open to all Scouts

At Monmouth Council, we want every Scout to have the opportunity to attend camp, regardless of financial barriers. If a Scout needs a little extra support, Camperships may be available, thanks to generous donations to the Council.

Alex Roberts Campership Fund

The Alex Roberts Campership honors Alex’s legacy by helping Scouts who show leadership, community spirit, and a strong commitment to their faith.

Eligible Scouts must have demonstrated:
- Leadership within their Troop and community.
- A helpful spirit towards others.
- A dedication to faith and service.

Health and Medical Forms

For the safety of all campers, completed medical forms are required for Anyone attending camp. The forms should be submitted prior to arrival at camp.

BSA Annual Health and Medical Record 680-001 Required for all campers.
Prescription Medication Form Must be completed for each medication.
Special Accommodations Now in Tentaroo Registration: For campers with specific dietary or medical needs. Please submit this as early as possible to ensure we can make the necessary arrangements.

Helpful Tips:

Plan Ahead: We strongly encourage all campers and leaders to complete forms early to avoid last-minute stress.
Medications: Don’t forget to bring any prescription medications with the Prescription Medication Form.
Special Requests: Any dietary restrictions or special needs should be communicated well in advance using the Special Accommodations Form.
